• MO DHSS has selected HabitNu to serve as the sponsoring hub to enable interested organizations to deliver the National DPP

• HabitNu will help with recruitment, data collection and reporting, program delivery, as well as submission and reimbursement of insurance claims.

• Partenering organizations can use HabitNu’s CDC recognition status, National DPP delivery platform, and Medicare supplier credentials to deliver and bill for National DPP services as they work to achieve full CDC recognition.
